The greatest source of introducing bugs in to a data driven dynamic system is this: Yourself. Confusing yourself actually.. or other people also working on the code. No matter how many design docs, meetings, huddles, white board sessions you have - you'll still confuse yourself, or be confused by somebody else. :)
